Mobilized Autologous Stem Cells in the SCI Treatment

It was a prospective control randomized double blind trial approved by the Scientific Board and Ethical Committee of the Russian State Medical University. The method was compared to conventional therapy of consequences of SCI. Evaluation of ASIA, FIM scales, MRI, ENMG, EEG, urodynamic tests, immunochemical tests of blood and SCF were done to assess effectiveness of the method. The sample of 82 SCI cases was split into two arms: 1 arm (42 cases) received mobilized autologous stem cells (MASC); 2 (control) arm (40) received conventional therapy and intrathecal infusions of normal saline. Intrathecal transfusions of MASC marked CD34+ were used in complex therapy of SCI. To mobilize CD34+ cells, extracted from bone marrow and parenchymatous tissues to peripheral blood, gran-ulocyte colony-stimulating factor was administered. MASC were harvested by flow type separator at the highest concentration. Received MASC were then frozen in tubes of liquid nitrogen, thawed and washed with 9%NaCl before clinical use. MASC preparation 6-8,5 x 106 CD34+ was mixed with the carrier (1:3) and transfused intrathecally High effectiveness of MASC transplantation compared to control arm (p < 0.05) was convincingly confirmed. The results were confirmed by ASIA and FIM evaluation. MASC transplantation in SCI can be viewed as the method of choice, effectiveness of which constitutes 75.1%. The suggested method of SCI treatment permits to partially restore damaged functions and to improve the quality of life and social adaptation of the patients.
